“Did I take it, or did I just think about taking it?” — the question this log exists to retire.
The ADHD Medication Tracker is a 14-page printable LOG — and only a log. You copy each medication’s name, dose and times exactly as they are printed on your own prescription label, and you tick a box when a dose is taken. The paper remembers, so your head doesn’t have to.
How the log works:
1. My Medication List — one row per medication, copied from the label on the container (the example rows use clearly fake placeholders — Medication A, Medication B — because your real rows come from your labels, never from a template).
2. Daily Log — one day per print: medication, time on the label, a Taken tick, the time actually taken, notes. Print a month at once and keep the stack where the morning happens.
3. Weekly Grid + Monthly Overview — the pattern of ticks at a glance: one box per day, up to four medications across a month. For your eyes only — a blank box is data, never a grade.
4. Refill Log — the refill dates YOU set, held in one place, so a renewal never ambushes you.
Also inside: a Label Details page (pharmacy details in your writing, where the log lives, a label-copy checklist for when a label changes), a Notes for My Next Appointment page (things to mention, written down while you think of them — take the log with you), a Weekly Log Review (is the LOG working — sheet in the right room, pen where it should be), spare daily sheets, and a notes page.
This is a log grid, the way the ADHD Bill Pay + Deadline Tracker is a log grid: one records doses already prescribed, the other records bills you organise — and neither one gives advice. It is not a reminder app (it never pings, and says so) and it is not a health record (no vitals, no symptom pages, nothing to report). Follow your prescriber’s instructions; this tracker only records what you already take.
Details: 14 pages, US Letter (8.5 x 11″), print-ready PDF. Fully undated — start any day, print the log pages forever. Calm slate-blue design with generous rows and room to write.
This is a planning tool, not medical advice. Instant download. No shipping, no waiting. For personal use.
